This is a legal document that affects my rights. I acknowledge that participation in the HOSTPLUS Health Wealth Program (“Program”) is a recreational activity that involves a significant degree of physical exertion and a degree of physical risk. I am participating in this Program for the purpose of recreation, enjoyment, and leisure.
I acknowledge HOSTPLUS has provided me with the opportunity to participate in this Program which includes physical and non physical training components. As a registered participant, I willingly assume the risk of participating in the exercises, training program, and all other activities in this Program. I understand that the Richmond Football Club and HOSTPLUS have not been provided with all relevant and necessary information about my health and capacity and therefore are not able to fully appreciate the risk of harm or injury to me in providing instruction and in allowing me to participate in this activity.
I acknowledge that Richmond Football Club and HOSTPLUS have recommended that I participate in a health check prior to commencement of the training program, and if I choose to participate in this health check, all medical information contained is private and confidential and will not be made available to either the Richmond Football Club or HOSTPLUS.
In addition, I acknowledge the fact that Richmond Football Club and HOSTPLUS will have access to my contact and personal details, for the purposes of enrolment in the program. HOSTPLUS will also need to use this information to communicate with me for the duration of the program.
I understand this information will not be used for any other purposes. I (or my parent or guardian) willingly provide the following waiver:

(a) I do not hold the Richmond Football Club or HOSTPLUS, their employees, agents or servants legally civilly responsible for injuries (including or limited to death) I suffer on their premises or using their equipment through participating in this Program.

(b) I undertake not to take legal action against the Richmond Football Club or HOSTPLUS, their employees’, agents or servants for any claims, costs, damages, or other liabilities they may have for injury suffered by me as a result of participating in this Program and I acknowledge that this waiver represents a legal release and discharge of legal responsibility to the Richmond Football Club and HOSTPLUS or their employees, agents or servants. It is provided in exchange for the goods and services purchased by me or on my behalf.

(c) I have been informed and I accept that I may also be assuming a role involving responsibility for the safety of others participating in the activities of this Program. This means I will accept responsibility for the safety of another person and if an injury occurs as a result of my careless act, omission or negligence then I fully assume responsibility for any harm done and I do not hold the Richmond Football Club or HOSTPLUS concurrently responsible.

(d) I warrant not to participate in the Program while intoxicated or affected by drugs.

(e) I acknowledge that any intentional damage to Richmond Football Club property inflicted by myself, will be covered by myself.

(f) I acknowledge the fact that Richmond Football Club or HOSTPLUS reserve the right to undertake assessment of participants suitability to undertake the program and if deemed unsuitable refuse the right to participate in the program.

(g) I agree that I will apply the correct and safe methods of using equipment when participating in the Program.

(h) I acknowledge that any activities I participate in outside of set Program times and utilising RFC facilities, will be undertaken at my own risk.
